Purpose of research - to explore the features of systemic and local antitumor immune response in skin melanoma patients depending on the presence of the primary tumor ulceration and to determine the immunological criteria of predicting the course of disease to optimize treatment. Methods of research - immunological, method immunohistochemistry, statistics. Equipment: histoprocessor, microtome, microscope, centrifuges, pH-meter. The immunohistochemical method has been used to investigate the immunophenotype primary tumor inflammatory infiltrate lymphocytes of 20 localized melanoma skin patients with depending on presence of melanoma ulceration. It has been established that patients with nonulcerated and ulcerated melanomas have the same primary tumor immunological profile on the studied parameters (CD3, CD8, CD20, CD45RA, CD45RO). The lymphoid infiltrate of primary tumor consists predominantly of T cells, most of which belong to the subset of cytotoxic T-lymphocytes, and they are represented as naive cells and immunological memory cells. An increase in the proportion of cytotoxic T-lymphocytes and/or memory T-cells in primary tumor immune inflammation lymphocytes is associated with a favorable disease course. The highest density of immunological memory cells in tumor tissue is observed in the presence of ulceration in patients with the best clinical response to combination therapy, including a 12-month course of adjuvant lowdose а2b-interferon therapy.
