We examined 120 liquidators of the Chernobyl accident with chronic nonspecific lung diseases in the remote period after the disaster, and a 50 persons of comparison group, patients with chronic nonspecific bronchopulmonary disease without formed nosological form in bronchopulmonary disorders. It was established that in the liquidators compared with the group of nosological control there were significantly worse indicators of the degree of shortness of breath and frequency of exacerbations per year, more clearly a violation of physical activity tolerance, higher comorbidity, a positive correlation between the dose of ionizing radiation and the values of FEV1 and FEF75 in liquidators, regardless of the presence of chronic obstructive pulmonary disease. A significant violation of the ratio of pulmonary volume was found, which indicates a decrease in the elasticity of the lungs, more pronounced respiratory disturbances due to pulmonary hyperinflation. A positive correlation was established between the dose of ionizing radiation and the index of intracranial pressure in the liquidators (r = 0.1494, p <0.05), between the radiation dose and the relative length of telomeres in the liquidators (r = 0.407, p <0.05). In peripheral blood lymphocytes there was a decrease in the number of CD34 + 38-cytoplasm cells and SD34 + 38-early precursor cells, both in patients with COPD and without broncho-pulmonary disease, and correlation between radiation dose and the number of monocytes CD45-14 + (r = -0.1696, p <0.05) and lymphocytes CD45 + 14- (r = 0.22, p <0.05) of peripheral blood. A negative correlation was found between the number of TCR-variant lymphocytes and CD3-DR + activated lymphocytes (r = -0.227; p <0.05), histone levels of H2AX and CD3-DR + activated lymphocytes (r = -0.3193; p < 0.05). The recommendations for the examination and treatment of persons who have been exposed to ionizing radiation and inhaled radionuclide inputs have been developed taking into account the correlation between the functional status of the bronchopulmonary system and the distant effects of irradiation in the immune system.
