Objective: to determine radiation-induced changes in gene expression, genomic instability parameters and radiation aging as markers of non-tumor diseases in Chornobyl clean-up workers in the remote period after exposure. Methods of research - immunological, molecular genetic, cultural. Results. Radiogenic changes in the expression of genes-regulators of apoptosis, immune inflammation, adhesion and angiogenesis in the remote period after irradiation have been established. Dose-dependent down-regulation of BCL2, SERPINB9, CDKN2A, and STAT3 genes was maximal at doses more than 500 mSv. It was established correlation between BCL2, SERPINB9, TP53 gene expression and exposure dose, supression of CLTN2 gene expression, MCF2L dose-dependent overexpression, changes in the activation of IL1B, IFNG, TNF, gamma-H2AX and Cyclin D1 expressions, TERT overexpression, the significance of micronuclei of buccal cells as biomarker of irradiation. Established association of cardiovascular pathology with changes of CDKN2A, CLSTN2, CSF2, GSTM1, IFNG, TERF1, TNF, VEGFA gene expression; cerebrovascular diseases with CDKN2A, CLSTN2, CSF2, IFNG, ILB, TERF1, VEGFA; respiratory pathology with CLSTN2, GSTM1, SERPINB9, TERF1, TERF2, TNF, TP53, VEGFA; endocrine diseases with changes of expression GSTM1, MCF2L, VEGFA, TCR, Cyclin D1, histone gamma-H2AX.
