Background and Goal of Study: To improve the efficiency of treatment of patients of different age groups subject to surgical intervention in various surgical areas or require urgent and intensive care, by substantiating the most effective methods of anesthesia, developing, improving and introducing new organoprotective technologies of intensive care at the stages of treatment based on the study of clinical , biochemical, pathophysiological changes in organs and systems of the organism. Object of research: changes of cognitive functions at births and birthdays during labor pain analgesia; the course of a burn disease in the course of intensive therapy of burns of various degrees of gravity; organic pathology of the central nervous system (purchased hydrocephalus and cerebral palsy), in which total intravenous, inhalation and combined anesthetic regimens of thromboprophylaxis are used in patients with moderate risk of thromboembolic complications in routine abdominal surgery; the course of the perioperative period with operations on the open heart under conditions of different variants of perfusion support; polytrauma, complicated blood loss. The essence of the study: the study of cognitive function, the level of sex hormones (estradiol, progesterone), pro- (IL-6, TNF) and anti-inflammatory (IL-10) cytokines, depth of anesthesia, quantitative characteristics of pain; indicators of homeostasis, hemodynamics at births and birthdays; endpoint of treatment (cognitive function on the 28th and 46th day of the postnatal period); study of clinical manifestations of bursal encephalopathy as a result of organ dysfunction in victims with severe and extremely severe thermal trauma; study of structural and functional changes of the brain, hemodynamics, cerebral blood flow, brain metabolism, acid-alkaline state, pain syndrome in children with organic pathology of the central nervous system; study of hemostasis, inflammation, hemodynamics in patients with moderate risk of thromboembolic complications during planned surgical interventions in abdominal surgery and undesirable effects in the treatment of thromboprophylaxis by various anticoagulants; study of the parameters of oxygen exchange, hemodynamics, hemostasis and inflammatory response in patients with artificial blood circulation on the background of various variants of the initial volume of filling of the device of artificial blood circulation and ultrafiltration of blood at cardiosurgery operations; study of indicators of central and peripheral hemodynamics, oxygen and water-electrolyte exchange, acid-alkaline state, hemostasis, inflammation and polyorganic insufficiency in patients with polytrauma. Methods of research: clinical, laboratory, immunological, instrumental, statistical: indicators of clinical signs of organ disorders, psychoneurological state, central, cerebral and peripheral hemodynamics, respiratory, redox processes, endothelial dysfunction, hemostasis, systemic inflammatory response, markers of myocardial damage, exchange oxygen, hypoxia, acid-alkaline state, immune status, functional and morphological state of the brain, pain.
