Department of psychiatry popup.description2 For the first time in patients with acute pancreatitis the basic functions of the intestine (motor-evacuation, absorption, barrier) have been learned. The intestinal insufficiency syndrome has been proved to exist in severe form of the disease. It is shown the conductive role of ischemic and reperfusion injury of the intestine in patient with AP. Pathogenetic therapy of IIS has been improved and proved to be effective in the treatment of acute form of the disease. Simple total scale of diagnostic severity of AP in first 3 days of the disease were created. The optimal terms, indications and choice of the method of operative intervention in AP were defined. The comprehensive study of the functions of the small intestine revealed its associated disorder, which requires multicomponent therapy with mandatory inclusion of drugs with antioxidant and antihypoxic action. The developed protocol of the treatment had positive influence on the early recovery of the studied functions of the intestine and the possibility of carrying out early enteral nutrition. With the help of a set of step-by-step statistical methods, a total score scale of differential diagnosis of severity of the disease was created which included eight indicators with diagnostic sensitivity and specificity 94.8% and 91.5% accordingly .Optimal terms for the opened operative interventions were defined.
