The possibility of reconstruction of the vascular bed by obtaining unidirectional blood flow, rebuilding resistance anastomoses, connecting vascular networks of adjacent perforated vessels, arteriogenesis has been experimentally proved. Determination of the terms and justification for the use of extended combined flaps on the basis of key and perforated vessels in the clinic in the treatment of severe deformations made it possible to close much larger wound areas with a good cosmetic effect. An algorithm for determining the sequence and stages of the formation of extended combined flaps on the basis of key and perforated vessels in the treatment of patients with scar deformations of different localizations was developed.
