The aim of research was to develop pathogenetically based treatment and methods for the prevention of non-alcoholic fatty liver disease in patients with metabolic disorders based on the investigation of genetic determinism and inflammation markers in the liver tissue. Research methods - clinical, laboratory, biochemical, enzyme immunoassay, molecular genetic and instrumental diagnostics of non-alcoholic fatty liver disease. The features of phenotypic and metabolic disorders in patients with NAFLD were studied. The profile of pro-inflammatory markers and their relationship with metabolic disorders in NAFLD was evaluated. The levels of miRNA-34a, miRNA-122 were investigated and their role in the development and progression of NASH in NAFLD patients was determined. The possible associations of the polymorphism of the PPARG2 (rs1801282), ADRB2 (rs1042713), ADRB3 (rs4994), and FABP2 (rs1799883) genes with metabolic disorders and liver steatosis degree were analyzed. The effect of standard lifestyle modification on the dynamics of biochemical parameters and markers of systemic inflammation in NAFLD were evaluated. The criteria for prescribing personalized treatment for patients with NAFLD combined with metabolic syndrome are defined. A new technology of medication treatment was developed based on systemic inflammation markers and its effectiveness in patients with NAFLD combined with MS was evaluated.
