Subject of study: Surgical treatment of patients with adrenal tumors. Aim of the project: Improvement of the results of surgical treatment of patients with adrenal tumors by developing and implementing a multidisciplinary approach. The multidisciplinary approach includes: a) arterio - and phlebography to determine blood supply variants; b) endovascular selective coagulation of the arteries and central vein of the adrenal gland under X-ray control; c) treatment (open or laparoscopic surgery). The practicability of the use of preoperative endovascular selective electrocoagulation occlusion of the adrenal vessels under X-ray control in the prevention of intra- and postoperative complications was investigated and proved. A therapeutic and diagnostic algorithm for the management of patients with adrenal neoplasms has been developed, including controled selective electrocoagulation occlusion of the adrenal vessels under X-ray control. X-ray endovascular anatomy and features of blood supply to the adrenal glands were studied. The endovascular occlusion technique under X-ray control of the veins and arteries of the adrenal glands has been elaborated and implemented in clinical practice. The possibility of safe performance of laparoscopic adrenalectomy for hormonally active tumors after endovascular selective electrocoagulation occlusion of the adrenal vessels, which allows to start the operation not with isolation and ligation of the adrenal vein, but with the mobilization of the tumor without negative period action has been proved. The introduction of endovascular technologies under X-ray control in the algorithm of surgical treatment of patients with adrenal tumors has significantly improved the results of treatment, reduce the incidence of complications in the perioperative period, provide early rehabilitation of patients and reduce the length of stay of patients in the hospital, which in particular increases the economic efficacy of the treatment.
