It has been established that population migration due to the armed conflict in eastern Ukraine has led to a geographical redistribution of HIV population from the east (Donetsk and Lugansk regions) to other regions of the country. It has been determined that genetic variants of ancient HIV infection are moving in Ukraine, not a new wave of the HIV / AIDS epidemic. It has been found that more viral lines are moving towards places with a high proportion of people who use injecting drugs (IDUs) and engage in risky sexual behavior. The spectrum of mutations of resistance (MR) of HIV to antiretroviral drugs (ARVP) and the frequency of their formation in HIV-infected patients of different age groups were established. It was shown that the duration of antiretroviral therapy (ART) and the age of patients had no significant effect on HIV resistance to ARVP. The genetic barrier (GB) of ARVP was crucial: against the background of taking drugs with low GB, the incidence of HIV MR was 5.3%; administration of drugs with high GB was accompanied by the formation of HIV MRI less frequently - in 1.1% of cases. For the first time, it has been proven that the bio-behavioral characteristics of male patients are a factor associated with an increased risk of HIV MRI. It has been determined that in Ukraine, against the background of scale-up of ART, it is not practiced to assign HIV-positive patients to mono- or bi-therapy, which is a significant achievement in the fight against HIV. At the same time, ARVP deficiency, problems with the maintenance of patients under the supervision of the dispensary and the achievement of virological efficacy of treatment were identified in many regions. The results obtained are of significant theoretical importance as the main source of information on trends in the formation of HIV resistance in the expansion of ART in conditions of active migration processes triggered by hostilities. On the basis of the scientific data obtained, for the first time in Ukraine, a National Concept for HIV Resistance Monitoring was developed and will be submitted for approval by the Cabinet of Ministers of Ukraine. The results of the researches give an opportunity to solve pressing issues of practical importance: on the basis of evidence-based medicine they allow to substantiate the choice of classes of ARVP in the conditions of expanding the scale of therapy, as well as the expediency of strengthening preventive measures for HIV infection not only among IDPs, but also among the general population of IDPs.
