DISCIRCULATORY ENCEPHALOPATHY, BRAIN STROKE, METABOLIC SYNDROME, CLINIC, PATHOGENESIS, TREATMENT Object of the study - patients with cerebrovascular disorders in the form of cerebrovascular pathology (CVP) and cerebral ischemic stroke on the background of metabolic syndrome (MS). Purpose of the study: to determine the features of the course of cerebrovascular disorders in patients with metabolic syndrome. 180 patients with CVP in the form of DE and ischemic ВS on MS background were examined. The control group consisted of 90 patients with CVP without MS. Characteristic features of DE on the background of MS were more severe and disease, with more pronounced neurological and cognitive deficits, more aggressive course of hypertension, the presence of lacunar foci (according to neuroimaging methods), signs of systemic lesions both cerebral and peripheral vascular system, and periphery . It is shown that cytokine linkage of inflammation and angiogenesis factors play a significant role in the pathogenesis of cerebral ischemia. The revealed peculiarities of CVP in the background of MS indicate a significantly higher risk of MI in this group of patients. The course of ischemic BS in MS was accompanied by more pronounced disorders, the process of restoration of lost functions was slower. All this confirms the need for early detection and adequate treatment of MS patients. Treatment tactics require: correction of cardiovascular risk factors, improvement of cerebral hemodynamics, impact on endothelial dysfunction and inflammation, correction of neurocognitive disorders, neurological symptoms and syndromes.
