Purpose: to study the general links in the mechanism of the pathogenesis of osteoporosis, osteoarthrosis and spinal osteochondrosis in the patients and experimental rats. Research methods and apparatus: clinical, bone densitometry, experimental animal modeling, biochemical, histological, electron microscopy, statistical. Theoretical and practical results: It is proved that negative remodeling of the subchondral bone is a common link in the development mechanism of arthrosis, spinal osteochondrosis and osteoporosis. Abnormal OPG and RANKL expression was detected in articular cartilage chondrocytes. A relationship was established between low BMD on postmenopausal women with deficiency and insufficiency 25(OH)D: 100% — at spinal osteochondrosis, 97.3% — gonarthrosis and coxarthrosis. It has been shown that a decrease in serum magnesium on postmenopausal women with low BMD is associated with a low level of 25(OH)D. The patients with low BMD and arthrosis had increased activity of the sympathoadrenal system. Novelty: New knowledge about the role of subchondral bone in mechanisms of development of osteoporosis, osteoarthrosis, spinal osteochondrosis. The decrease vitamin D is a common link in the development of structural and functional disorders of cartilage and bone. The role of magnesium in the pathogenesis of osteoarthritis, spinal osteochondrosis and osteoporosis has been proved. Increased activity of the sympathetic division of the autonomic nervous system on patients with low bone mineral density and arthrosis was first demonstrated. Implementation efficiency: Recommendations for improving the diagnosis and prevention of patients with osteoporosis, osteoarthrosis, and spinal osteochondrosis have been developed.
