Aim: to study the role of adipokines in mechanisms of development of cardiovascular complications in patients with type 2 diabetes with NAFLD. Methods: clinical, biochemical, immunological, statistical. Weight gain is accompanied by an increase in NOMA, FFA, and leptin index, which correlate positively with fractalkine, clusterin, vaspin, and vice versa with omentin.Patients with type 2 diabetes mellitus with NAFLD have a direct relationship between adiponectin and TG, which indicates the progression of atherogenic dyslipidemia with decreasing adiponectin levels. A direct relationship was found between resistin levels and IL-1β and IL-6. Resistin unidirectionally correlates with some lipid fractions, confirming its role in atherogenesis. Hyperhomocysteinemia can be considered a marker of fibrotic changes in liver tissue and a marker of cardiovascular risk.Against the background of dapagliflazine and metformin therapy, a decrease in BMI, non-invasive liver fibrosis, improvement in carbohydrate and lipid metabolism were detected. Average levels of leptin in patients with type 2 diabetes with NAFLD increased.
