The results suggest that in women with MS with BMI less than 35 genetically determined features of the functioning of hypothalamus-pituitary-adrenal system is the trigger mechanism of fat deposition by visceral type and the onset of adipose tissue dysfunction, which in turn may have a gonadotoxic effect. At the same time in MS on the background of obesity in women of reproductive age is secondary to the suppression of the function of hypothalamus-pituitary-gonadal axis, mainly due to hyperleptinemia. Clinical variants of MS were found for the first time to have different biochemical adiposopathy patterns: in the group with MS for normal BMI there was the most pronounced adiponectin deficiency, which indicates the highest risk of cardiometabolic complications in these women, at the same time in the group of women with obesity despite hyperleptinemia, adiponectin levels were comparable to those in women without MS. In addition, significant correlations were found between adiponectin, AMH, FSH, cortisol, and androgen levels. Differences in vitamin D levels in women with different clinical varients for MS were first investigated. It was found that in MS with normal weight, there was the most pronounced deficiency of vitamin D. In this case, the level of Vit D showed a strong direct correlation with the level of adiponectin. It was found that women who underwent successful treatment of morbid obesity with bariatric surgery at the perimenopausal age had significantly more favorable course of menopausal transition: less pronounced vasomotor symptoms, lower incidence of abnormal uterine bleeding, lack of responsiveness, absent endometrial cancer risk.
