Purpose: determination of ultrasound, cytological, immunocytochemical features of radioiodine-resistant metastases of papillary thyroid cancer, development of software for automation of the basic stages of the algorithm for determining the level of asymmetry of cell nuclei on the material of puncture biopsies of thyroid neoplasms, assessment of the structure of the uterus, ovaries and breast and reproductive system disorders in patients after radioiodine therapy with long-term follow-up. Methods: ultrasound, cytological, immunocytochemical, morphometric, statistical. Studies have shown that ultrasound revealed metastatic lymph nodes of the neck allow for further immunocytochemical and cytological studies, which can increase the efficiency of diagnosis of metastases of differentiated thyroid cancer and to develop treatment tactics. A statistically significant difference in the expression of thyroid peroxidase and thyroglobulin in FNA smears of radioiodine-resistant and radioiodine-avid metastases of papillary thyroid cancer has been shown on the basis of which a new method of preoperative prediction of radioiodine resistance and efficacy of radioiodine therapy was developed. As a result of the work, the algorithm for preoperative prediction of radioiodine resistance of thyroid papillary carcinoma was first proposed, based on the detection of cytological and immunocytochemical features of thyrocytes in the FNA smears.
