The purpose of the work is to improve the results of treatment and quality of life of patients with oral cancer and oropharynx by optimizing the methods of replacement of postoperative defects with vascular grafts. Research methods and apparatus: clinical, clinical and laboratory, radiological, pathomorphological, statistical. The methods of plastic replacement of half defects of the tongue with the use of a submental flap and a radial flap of the forearm have been improved. The technique of reconstruction of the tongue after subtotal resection with a modified flap of the large pectoral muscle with a vertically oriented skin part was developed, and a radial flap of the forearm was improved. Methods of replacement of deep defects of the mouth cavity with the use of modified flap of the large pectoral muscle with horizontally oriented skin part and skin-muscle flap of the sternocleidomastoid muscle with perforation of the upper thyroid artery were developed. Methods of plastic elimination of surface defects of the oral cavity bottom with skin-fat nose-lip flap and skin-muscle flap of platyism have been improved. Methods of plastic replacement of defects of the mucous cheek, retromolar area and oropharynx of the fascial-aponeurotic temporal-parietal flap and skin-fascial flap of the supraclavicular artery have been developed. The application of the developed methods of replacement of postoperative defects of the oral cavity and oropharynx made it possible to restore nutrition naturally in 92% of patients after 10-14 days after surgery and to achieve high rates of functional rehabilitation and quality of life of patients.
