Yanovsky National academy of medical sciences of Ukraine" popup.description2  Object – 355 patients with pulmonary and pleural tuberculosis (TB) who underwent surgical treatment using minimally invasive videothoracoscopic techniques. The purpose of the work is to increase the efficiency of surgical treatment of patients with pulmonary and pleural tuberculosis. Methods – clinical-instrumental, laboratory, cytological, microbiological, pathomorphological, statistical. It was found that VATS had a shorter average duration of surgery – (75,1 ± 22,3) minutes, intraoperative blood loss – (85,4 ± 1,6) ml. The course of the postoperative period in VATS was characterized by a low level of intraoperative complications (3,8 ± 1,7) % of cases and postoperative complications (13,1 ± 3,0) % of cases. More often VATS was performed in tuberculous lung lesions 67,9 %, of which in pulmonary tuberculomas – 68,8 %, more with moderate activity of the process – 37,5 %. The technology of conducting the postoperative period after video-assisted resections has been developed, the effect is to reduce the frequency of postoperative complications by 8,5 %, to reduce the duration of narcotic analgesics by 3,0 days, to reduce the length of hospital stay by 7,2 days. A method of video-assisted resection for pulmonary and pleural TB with pleural obliteration was developed, which allowed to reduce the frequency of postoperative complications by 14,5 %, reduce the length of stay in the hospital by 5,8 days, increase the overall effectiveness of treatment by 14,5 %. A new method of video-assisted lung resection in patients with multidrug-resistant TB has been developed, which has reduced the incidence of postoperative complications by 6,6 %; reduce the length of stay of the patient in the hospital by 5,6 days; increase the overall effectiveness of treatment by 6,6 %.
