Methods and equipment - clinical, surgical, radiological, laboratory; videothoracoscopic complex "Contact", devices of artificial ventilation of lungs "Dragger", "Angstrom", electrodiathermic installation EK-300M1. It was found that at the stage of completion of treatment of the primary case of tuberculosis in most patients with recurrences of the disease were determined densitometric parameters, which corresponded to high (60.7%) or medium (32.1%) activity of a specific process, which totaled 92.9% of the total number of patients with recurrence of tuberculosis. Immediately upon recurrence of tuberculosis in all cases, pathological changes with a density below 20 HU were found, which corresponds to the high activity of a specific process. It was found that the aggravated course of the intra- and postoperative period in patients operated on for TB was accompanied by deviations of densitometry values from the conditional norm. At the increased (over -730 HU) density of a parenchyma obliteration of a pleural cavity and interlobular cracks, fibrous changes of a root of a lung, at the lowered density (below -860 HU) - bleeding and insufficient tightness of a suture of a parenchyma which together aggravated and followed contributed to the overall unsatisfactory results. Means of prevention of the burdened course of the intra- and postoperative period consist in the use of rational means of pneumolysis, separation of the interlobular fissures and the performance of the suture of the pulmonary parenchyma. The developed method of separation of interlobular fissures when performing resections in patients with pulmonary tuberculosis provides effective hemostasis and tightness of the divided lung parenchyma, reducing the frequency of adverse postoperative period, and as a result, reducing the duration of postoperative stay in hospital from з 20,7 to 14,5 days.
