Aim – to study the rate of pulmonary sarcoidosis relapses in the follow-up period upon completion of methotrexate immunosuppressive therapy. Methods – clinical, computed tomography of chest, pulmonary ventilation and diffusion diagnostic tests. The number of clinical cure without sequalae cases has significantly increased in methotrexate 15 mg/week arm comparing to 10 mg/week arm (81,3 % and 42,4 %, respectively). An increasing of methotrexate dose to 15 mg/week has reduced the time to clinical cure ((10,1 ± 0,5) months and (12,8 ± 0,8) months, respectively, р ˂ 0, 02), indicating higher rate of sarcoidosis regression. It has been evaluated rate of pulmonary sarcoidosis relapses and developed a method of treatment of the release with methotrexate, decreasing the rate of relapse by 2,6 times and the incidence of adverse reactions by 1,5 times in comparison glucocorticosteroid therapy. Immunosuppressive therapy with methotrexate in doses of 10 and 15 mg/week has been characterized by acceptable tolerance. Immediate and distant outcomes allow to recommend the use of methotrexate15 mg/week in form of monotherapy for treatment of pulmonary sarcoidosis patients who have contraindications for use of glucocorticosteroids.
