The purpose of the work – to determine the peculiarities of the prevalence of isolated and haplotypic polymorphic variants of cytokine genes (TNF-α, TGF-β1, IL-6, IL-10, IFN-γ) and genes of the main histocompatibility complex (HLA-A, HLA-B, HLA-C, HLA-DRB1, HLA-DQA1, HLA-DQB1) in non-irradiated and irradiated patients with PCM in the remote post-accident period. Research methods - clinical, molecular genetic, immunogenetic, statistical. Priority groups were formed with the inclusion of 56 patients with PCM (irradiated 31) to study polymorphic variants of cytokine genes and genes of the main histocompatibility complex. Probable increase in the frequency of the genotype TGF-β codon 10 T / T (0.591, p <0.05) and the specificity of HLA-C * 06, HLA-DQB1 * 05: 01, HLA-DQB1 * 06: 01 in the group of patients (irradiated, non-irradiated) relative to population control indicates their unidirectional associative relationship with the risk of pathology and the possible role as independent immunogenetic factors predisposing to the development of PCM. The prevalence of haplotypes in irradiated patients (low (ATA), medium (ACC), high (GCC) producers), mainly corresponds to their prevalence in non-irradiated patients with PCM and controls. A significant decrease in the frequency of the IL-10-1082 / -819 / -592 GCC haplotype as a producer with a high level of secretion of the anti-inflammatory cytokine IL-10 was shown in the group of non-radiated patients. In non-radiated patients with PKM, an increased prevalence of HLA-A * 24 / B * 07 / C * 06 / DRB1 * 11 / DQB1 * 06: 01 / DQA1 * 01: 01 haplotype with the inclusion of HLA-A * 24 and HLA- B * 07 - markers of low immunological response, which may affect the activity and response strength of the immune system during treatment.
