Methods of research: clinical, cytologycal, cytochemical, hematological, immunological. The aim of the work is to increase the effectiveness of treatment of victims with dermal and deep burns by developing and pathogenetic substantiation of the transfusion program to optimize the wound process. It has been established that complications of burn disease are accompined by a change in the levels of all studied parameters associated with the state of general toxic effects in the clinic of the disease. Endotoxicosis expression, function decompensation of the natural resistance factors, lymphocite activity in patiets corellate with severity of burn disease clinical cource. The connection between the level of the investigated indicators and the severety of injury was established. It was established that the causes of the systemic inflammatory response in severe thermal trauma are severe and extremely severe endotoxicosis associated with accumulation of tissue destruction products, microbal toxins, decompensation of the system of natural detoxication, metabolic and immunological distress with the devepopment of the immune responce in the T-independent way, the peculiarities of the pro- and antiinflammatory cytokines dynamics, the formation of autoaggressive reactions, in wich maintenance toxins and toxin associated blood plasma are directly involved. The obtaqined data justifies new programs of transfusion treatment for patients with dermal and deep burns, depending on the injury severity and the stage of burns. Medical and social effect.
