For the first time, the immunophenotype of the leukemic stem cells (LSC) was studied in different clinico-prognostic variants of AML. Subclone of LSC in the "total pool" of blast cells was detected more frequently in myelomonocytic (M4) and monoblastic (M5) variants of AML and in AML with minimal differentiation (M0). LSC were not determined in promyelocytic (M3) and myeloblastic (M2) variants of AML wherein the major cell substrate is represented by the blat cells of higher differentiation grade (CD34-CD38-CD117-/+CD33+CD13+/-). The LSC identified by immunophenotype CD34+CD38-CD117-CD90-CD123+ in M4 and M5 variants of AML seem to be associated with lesser efficacy of therapy requiring the correction of the treatment schedule. The absence of LSC in M3 and M2 variants of AML, despite more aggressive clinical course of these variants, seems to facilitate the achievement of the remission at an earlier time.
