Purpose: to improve the results of treatment of patients with knee contractures on the basis of finding out the causes of contractures, development and improvement of methods for their prevention and treatment. Research methods and apparatus: clinical, radiological, laboratory, immunological, ultrasonographic, electromyographic, rheovasographic, morphological, histological, biomechanical, mathematical modeling, statistical. Theoretical and practical results: The functional state of the lower extremity muscles in patients with knee contractures was determined, the effect of muscle extensibility on the formation of knee contractures, features of metabolic reactions and dynamics of laboratory changes in these patients were studied. Changes in the stress-strain state in the area of "endoprosthesis-bone" in bone defects and the possibility of its normalization due to various plastic techniques were studied. Methods of endoprosthesis and postoperative rehabilitation in patients with knee contractures have been developed. The approbation of the developed techniques in the clinic is carried out, their efficiency is proved. Novelty: New links in the pathogenesis of joint contractures and features of their course depending on the severity of osteoarthritis. A differentiated system of prevention, surgical and conservative treatment of knee contractures in patients after injuries and endoprosthetics has been developed. A set of individual rehabilitation measures was created and implemented depending on the type of contractures and the reasons for their formation, which allowed in 85% of cases to obtain positive treatment results. Implementation efficiency: Recommendations for improving the prevention and treatment of patients with knee contractures have been developed. Field of use: medicine.
