Purpose:to improve existing and develop new methods of bioreconstruction of long bone defects in the surgical treatment of patients with bone tumors on the basis of experimental and clinical studies. Research methods and apparatus: analytical, statistical, morphological, finite element method, biomechanical. Theoretical and practical results: A meta-analysis of the effectiveness of the use methods of allograft prosthesis composite and megaendoprosthesis of post-resection defects of long bones in the case of their tumor lesion was performed. Several methods of surgical treatment of patients with malignant bones tumors have been proposed and improved. Their efficiency is proved by the method of mathematical modeling and biomechanical experimental researches. Domestic endoprostheses for allograft prosthesis composite have been developed. Clinical approbation of the developed methods is carried out. Novelty: the joint strength of the bone segmental alloimplant and the recipient bone in the case of allograft prosthesis composite was studied for the first time. It is proved that performing a step-cut osteotomy and appropriate fixation of a long bone can halve the level of mechanical stresses and increase bone strength in the area of contact of the alloimplant and the recipient's bone in the case of a step-cut osteotomy. Morphologically, it has been experimentally proven that in the case of using microporous polyethyleneterephthalate to restore the extensor apparatus of the knee joint in individual endoprosthesis of post-resection defects of the proximal tibia, tendon-like tissue is formed. A new material has been developed - glass-crystalline calcium phosphate material ASZ-5 - to replace small bone defects, its efficiency and biocompatibility with bone have been morphologically proven.
