The study subjects included blood and bone marrow cells, blood plasma from 22 patients with true polycythemia, 22 patients with essential thrombocythemia, and 10 patients with myelofibrosis. The aim of the study was to establish the influence of immunomodulatory cytokines and growth factors on the course of certain chronic myeloproliferative neoplasms (erythremia, essential thrombocythemia, primary myelofibrosis), in particular regarding the progression of the pathological process, the development of hemostasis disturbances, and to develop principles of pathogenetically based therapy. Research methods included general clinical, cytological, histological, cytogenetic, molecular genetic, coagulogical, statistical analysis. MULTISKAN enzyme-linked immunosorbent assay, analytical scales, pipette dispensers were used for this study. At the first stage of research an assessment of the diagnostic and treatment process for individual MPN on the basis of examination of patients with PV, ET, MF was performed and a study group was formed. A new method of molecular genetic examination of patients has been established, in particular, the determination of mutations in the CALR gene. Different frequencies of driver mutations were detected in certain subtypes of MPN. Work has begun on the introduction of a diagnostic algorithm in patients referred with suspected PV and ET to the clinic of the institute. Based on a study of molecular genetic markers of individual chronic myeloproliferative neoplasms, cytological and histological characteristics of hematopoiesis, the levels of proinflammatory cytokines and growth factors, their impact on clinical manifestations of MPN will be determined, the course and complications associated with ineffective hematopoiesis and disorders of the hemostasis system, which is a novelty of this study.
