Purpose: to improve the diagnosis and surgical treatment of patients with hematopoietic and metastatic lesions of the thoracic and lumbar spine. Research methods and apparatus: clinical, magnetic resonance and computed tomography, biochemical, immunological, histological, statistical, experimental modeling. Theoretical and practical results: An algorithm for selecting the scope of surgical treatment of spinal tumors has been developed, the use of which allows to select with a high probability for a patient with spinal neoplasm the appropriate amount of surgery to ensure a satisfactory treatment result. Using a method of mathematical modeling, biomechanical and morphological research, a model of a carbon-carbon composite endoprosthesis for interbody fusion was developed and its effectiveness was proved. Its use allows to improve the results of treatment for hematopoietic and metastatic lesions of the thoracic and lumbar spine. Novelty: For the first time, an algorithm for selecting the volume of surgical treatment of spinal tumors was developed and its clinical effectiveness was proven. For the first time on the basis of mathematical modeling and biomechanical researches the model of endoprosthesis from carbon-carbon composite for interbody spondylodesis was developed and its efficiency was proved. It has been proven for the first time that a carbon-carbon implant with a pyrocarbon coating does not disrupt the course of bone regeneration and has better osseointegration properties compared to carbon implants without a coating. A domestic interbody body prosthesis has been developed to replace the vertebral body. Implementation efficiency: The use of the developed algorithm and new methods and devices allows to improve the results of treatment of patients with hematopoietic and metastatic lesions of the thoracic and lumbar spine and reduce the cost of treatment.
