Objective: to study the vascular mechanisms for providing adaptive capabilities to children with secondary cardiomyopathies and blood pressure disorders taking into account the state of regulation systems, indicators of oxidative stress and homocysteine. Methods: clinical, instrumental, functional tests, laboratory. Results: Changes in indicators of regulation systems, free-radical-initiated processes and endothelial factors in children depending on the phenotype of connective tissue dysplasia, the state of the endothelium, microcirculation and adaptive heart capabilities were studied. Endothelial dysfunction is associated with early signs of cardiac remodeling and diastolic myocardial dysfunction. Unfavorable prognosis factors for the development of endothelial dysfunction accompanied by the initiation of free radical processes, an increase in the levels of endothelin-1, thromboxane, and a decrease in antioxidant protection, were determined. Capillary aggregation in adolescents with hypertension and secondary cardiomyopathy (SCM) occurs on the background of lowered levels of prostacyclin, lipoxin, and neutrophilic elastase along with increased levels of blood thromboxane and endothelin-1. An algorithm for examining adolescents to determine the amount of physical activity and a technology for optimizing their physical activity were developed. Novelty: For the first time, the relationship between the state of the endothelium, microcirculation, adaptive capabilities of the cardiovascular system and indicators of regulation systems and free-radical-initiated processes in children with hypertension and SCM was studied.
