The aim of the work is to substantiate and develop the pathogenetic concept of physical rehabilitation in degenerative-dystrophic diseases of the spine and large joints, as a strategy to prevent their progression and disability of patients. Research methods - scientometric analysis of modern provisions of physical rehabilitation in degenerative-dystrophic diseases of the musculoskeletal system, sociological, clinical, anthropometric, functional, biochemical, immunological, sonological methods of examination, mathematical and statistical methods of analysis of the results. For the first time the pathogenetic concept of physical rehabilitation at functional and structural disturbances of the musculoskeletal system is substantiated and developed, which is based on the assessment of physical, metabolic, structural-functional and adaptive features of the patient and allows to determine determinants of inhibition of degenerative-dystrophic diseases. An integral part of the concept are technologies that are strategically focused on improving the efficiency of recovery using a comprehensive and differentiated approach to filling programs with means and methods of physical rehabilitation. The introduction of pathogenetically oriented technologies of physical rehabilitation has shown a pronounced rehabilitation effectiveness, which makes it possible to prevent disability of patients.
