They differ from previous studies by the pathogenetic substantiation of the use of various anaesthesia schemes. Based on the conducted studies, the technique for the complex preoperative assessment of a clinical and functional condition has been developed and risk stratification of concomitant pathologies while selecting elderly and senile patients for aorto-coronary shunting has been performed. Algorithms for conducting anaesthesia in patients with coronary heart disease (CHD) have been developed. They can be used both when performing on-pump CABG and general somatic surgery in such a contingent of patients. A pathogenetically substantiated complex of therapeutic and prophylactic actions has been developed to prevent the development of acute renal dysfunction that may occur during on-pump CABG surgeries. As a result of the research, there has been offered a schema for the reasonable use of intraoperative autologous blood procurement and acute normovolemic hemodilution during on-pump CABG, which is grounded on of maintaining the minimum allowable haematocrit and haemoglobin levels, ensuring adequate protection of the myocardium from hypoxia.
