Purpose: to improve the results of surgical treatment of patients with coronary heart disease with reduced contractility of the left ventricle to increase the effectiveness of the operation. The relevance of this research is due to the large number of patients with coronary heart disease with reduced left ventricular contractility, unsatisfactory results of their treatment and ineffective methods of prevention. The complexity of research and methods proposed in the work is a combination of general clinical, laboratory, instrumental, morphological methods to identify pathogenetic mechanisms of reduced left ventricular contractility, detection of pathogenesis of complications to improve the effectiveness of treatment and development of methods for prevention of myocardial dysfunction. For the first time, on the basis of morphological studies, it was determined that reduced left ventricular contractility are caused by both necrosis of cardiomyocytes and the formation of rigid fibrosis in the necrotic zone.
