The effectiveness of new and improved existing methods of reconstructive and restorative interventions using local, regional and free flap patients for locally advanced oral cancer has been studied and the most effective flaps have been identified. It was found that to replace half of the tongue defects, the use of a free radial flap of the forearm compared to the regional submental flap significantly increases the functional result from (80.4 ± 8.1)% to (85.6 ± 9.3)%; p <0.05 and quality of life of patients from (69.6 ± 23.6)% to (83.3 ± 12)%; p <0.05, but increases the risk of complications in the donor area from 6.3% to 40.0%; p <0.01. The advantages of replacing subtotal defects of the tongue with a regional modified skin and muscle flap of the pectoralis major muscle compared to the free radial flap of the forearm are established: the risk of complications in the donor area is reduced from 42.0% to 7.9%; p <0.01; the duration of surgery is probably reduced by an average of 3.9 h, in the absence of statistically significant differences between groups in terms of functional status, quality of life of patients, the frequency of complications after transplantation of rags. The expediency and necessity of extended surgical interventions in patients with neglected tongue cancer - glosectomy with laryngectomy and plastic formation of the pharynx by the developed method of modified skin and muscle flap of the pectoralis major, as it allows to restore nutrition naturally in 94.1% patients. The advantages of using a modified skin and muscle flap of platysma compared to a local nasolabial flap to eliminate superficial defects of the oral cavity have been established.
