Research methods and equipment: clinical, clinical-laboratory, radiological, morphological, immunohistochemical, ultrasonic, radioisotope, genetic, statistical; DNA Sorb B (Amplisens, Russia), Nano Drop 1000 spectrophotometer (Thermo Fisher Scientific, USA) and 7500 Real-Time PCR Systems (Applied Brnsystems, USA). The frequency of hereditary mutations BRCA1 (5382insC, 185delAG, 4153delA, 300 T> G, 2080delA) and BRCA2 6174delT among 440 patients with serous ovarian cancer in Ukraine was studied by polymerase chain reaction. Hereditary BRCA1 mutations were found to be diagnosed in 12.7 ± 1.62% of patients with serous ovarian cancer. The ESGO (2019) algorithm for surgical treatment of patients with serous ovarian cancer has been introduced. Personalized methods of treatment of patients with serous ovarian cancer based on BRCA status have been developed. The incidence of complete responses to neoadjuvant platinum-containing chemotherapy was higher in patients with BRCA1 mutations (34% vs 4%, p <0.001). Rising body temperature and cisplatin during HTPEC (PCI - 17) are accompanied by increased metabolic acidosis (mean blood pH 7.29 ± 0.06). The protocol of support therapy after cytoreductive surgical interventions in patients with disseminated serous ovarian cancer has been optimized. For the first time, domestic recommendations for monitoring and preventive measures in relatives of carriers of hereditary mutations in the BRCA1 / 2 genes have been formulated.
