Objective: to establish diagnostic and prognostic criteria of thrombophilia in patients with stage VD CKD based on comprehensive clinical and biochemical study of procoagulant factors of hemostasis as well as parameters of coagulation and fibrinolytic potentials, and to evaluate their pathogenetic role in the development of comorbid conditions. Research methods: general clinical and biochemical investigations, enzyme-linked immunosorbent assay and mathematical methods of data processing and analysis. The role of hemostasis factors in the development of comorbid conditions was assessed in 88 patients with stage VD CKD treated by program hemodialysis. The study demonstrated negative influence of increased duration of dialysis therapy on natural anticoagulant mechanisms causing increased procoagulant activity. But the presence of residual renal function was found to be associated with positive effect on hemostasis indices (decreased levels of soluble fibrin, D-dimer, fibrinogen, and increased C protein level). The study results are indicative of activated procoagulant status of hemostatic system in patients with stage VD CKD associated with inhibited anticoagulant and fibrinolytic activities, thus with high risk of thrombotic events. During the study period, generalized coagulation status, overall hemostatic and fibrinolytic potentials in patients with stage VD CKD treated by program hemodialysis were established as well as the relationship between systems of coagulation, fibrinolysis and anticoagulation, possible use of hemostasis markers and parameters of hemostatic potentials to predict thrombotic complications.
