The aim of the work is to develop a model for predicting the risk of developing TB and an algorithm for diagnosing the disease in children by determining the most important predictors taking into account the profile of resistance of the pathogen to anti-TB drugs. Research methods: clinical, radiological, laboratory, statistical. Drug-susceptible (DS) TB was treated in 71.7% and drug-resistant (DR) TB in 28.3%. In 61.8% of the patients examined, bacterial excretion could not be confirmed. Bacterial excretion is more frequently detected in children aged 15-17 years (44.9%) and 10-14 years (31.8%). In recent years, there has been a 10.4% increase in the proportion of children with bacterial excretion and an 11.0% increase in the proportion of those with DR-TB. In children with bacterial excretion from DR-TB cases, the sum of the percentages of MDR-TB, XDR-TB and Rif TB accounted for 57.3% of all resistance. In cases DR-TB, widespread processes accounted for 45.3% versus 27.9% in DS-TB. In 29.0% of patients with multiple DR to 4 drugs was determined. MGM were effective in 47.3%, culture in 35.1%. Xpert MTB/RIF ULTRA was 52.5%, Xpert MTB/RIF was 42.5%. Bacterial excretion in children was more frequently confirmed by examination of operative material (100.0%) and sputum (45,2%), in children under 4 years of age BAL (44.4%) and GL (31.6%). A model for predicting the risk of localized TB in children with LTI was developed by identifying the most significant predictors, which improves prediction accuracy by 29.4%. The algorithm of diagnostics of TB in children taking into account the resistance profile of the pathogen to anti-TB drugs was developed, which allows optimizing the sequence of decision-making on the diagnosis and treatment of TB in children.
