Objective: to improve the prevention of irreversible damage to organs and systems in children with RH by determining the dynamics of functional disorders of the central nervous, cardiovascular, respiratory, kidney, liver, visual, inflammatory factors and metabolic disorders under the influence of complex therapy in the observation stages. Methods: clinical and paraclinical characteristics, ECG, ultrasound, blood creatinine level, glomerular filtration rate, spirography, ultrasound of joints, examination of the eyes (slit lamp), lipid profile, coagulogram, 25-hydroxyvitamin D blood, immunological markers of inflammation (ANA, ANA, DNA, compliment titer), quality of life assessment, statistical. Results: it was found that children with RH have potentially irreversible disorders of organs and systems, which in patients with JIA depend on the duration of the disease, in patients with SLE - on the activity of the process. Children with RH develop atherogenic dyslipoproteinemias, which progress while maintaining the activity of the process and the presence of comorbid conditions. An algorithm for phased observation of children with JIA and SLE has been developed in order to prevent the formation of irreversible damage to organs and systems. Novelty: for the first time the frequency and nature of permanent damage to organs and systems of children and adolescents with SLE and JIA have been determined. Prognostically significant signs of irreversible organ damage have been established: the level of antinuclear antibodies, circulating immune complexes, fibrinogen, complement, atherogenic factor, international normalized ratio, daily dose of glucocorticosteroids.
