Objective: to improve the results of treatment of diseases, deformities and injuries of the spine by identifying the main errors and complications that adversely affect the transpedicular fixation of the spine and to develop systematic measures for their prevention. Research methods: clinical, radiological, computed tomography, biomechanical (mathematical modeling by finite element method, kinematic modeling), biochemical, statistical. Theoretical and practical results: The use of navigation system and neuromonitoring in preventing complications of transpedicular fixation of the spine is justified. The stress-strain state of the lumbar spine with polysegmental transpedicular fixation was studied by the finite element method and the dynamic stimulation of its movements was studied using a musculoskeletal model with changes in the sagittal contour of the spine. The influence of connective tissue metabolism on the development of inflammatory complications has been studied and an algorithmic system for their prediction after transpedicular fixation of the spine has been developed. Novelty: for the first time with the help of modern research methods retrospective analysis of patients after surgery for diseases, deformities and injuries of the spine, mainly thoracic and lumbar, with the identification and systematization of prognostically unfavorable factors leading to errors and complications in transpedicular fixation of the spine. Implementation efficiency: The results obtained can be used for further research and development of sound guidelines for the diagnosis and prevention of complications when planning spine surgery using transpedicular fixation.
