The aim of the work is to increase the efficiency of diagnostics of structural and functional changes in the structure of bone tissue and bone marrow depending on the indicators of iron metabolism in children with acute leukemia at the stages of chemotherapy. Research methods - cytological, biochemical, biophysical, immunological, dosimetric, statistical. Criteria have been developed for the first time to increase the efficiency of diagnosing changes in bone structure in patients with acute leukemia by iron metabolism (erythron status, ferritin level, iron, serum transferrin, sideroblasts in bone marrow), metabolic parameters of osteoblastoma, stroma in serum, bone density), and hormonal regulation (osteocalcin, cortisol, pituitary thyroid-stimulating hormone (TSH)) before and after treatment. Hypocortisolemia in patients with the onset of acute leukemia was accompanied by decreased activity of bone marrow stromal fibroblasts and shorter survival (2,3 times).An unfavorable prognosis was observed in patients with serum ferritin levels above 500 ng/ml, alkaline phosphatase activity above 600 U/l, bone density below 65 um.od. There is a direct correlation between serum TSH levels and oxyproline and proline in urine; between TSH levels in children with low bone density and higher iron levels 27 μmol / L; between serum iron and alkaline phosphatase; between qualitative changes in blood monocytes and the annual dose of internal radiation of the child. Irradiation doses of children with acute leukemias who fell ill from 1996 to 2010 were (5,37 ± 1,23) mSv; in children diagnosed after 2010 - (3,66 ± 0,11) mSv. There is no relationship between the identified values ​​and the radiation doses of children.
