It was revealed that platelet-enriched fibrin and hydrogel combination enhances the expression of chondrogenic marker genes by nucleus pulposus cells of rat intervertebral discs, active formation and viability support of chondroblast-like cell colonies. The obtained results indicate that the use of these matrices might be beneficial for the prevention of epidural adhesions after lumbar microdiscectomies. The evaluation of both clinical and functional patients’ status dynamics after the repeated operations due to the recurrence of intervertebral disc herniation showed the significant improvement in the shortest possible time (within the first 2 days post-operation) regardless the intraoperative epidural insertion of “Nubiplant” hydrogel. Thus, low back and radicular pain was reduced from the excessive to mild, the degree of pain-related disability dropped from the severe to minimal, while the strength of the lower limbs increased. In two days after the operation and in 3, 6, and 12 months as well as in the remote period, these parameters have been remained within the specified limits, and did not differ significantly in hydrogel-inserted and conrol (with no hydrogel) patient’s groups. At the same time, the positive effect of the operation lasted longer in the group of hydrogel-inserted patients. Therefore, it has been found that intraoperative epidural injection of “Nubiplant” hydrogel allowed to reduce significantly the frequency of MRI signs of epidural edema and hemorrhage in the early postoperative period as well as the frequency of MRI signs of epidural fibrosis in the late postoperative period.
