This evaluation showed that MVD was superior in both primary and recurrent cases. We developed and validated forms for survey and data collection which help to summarize information about each particular case of classic trigeminal neuralgia management. Those forms are holistic for integration of objective case data with subjectively reported survey questions regarding patient compliance, wellbeing and disease-related quality of life. This study also uncovered anatomical and topographical peculiarities of neurovascular relationships in cases of secondary trigeminal neuralgias due to tumor compression. Thus morphological foundation was set for further optimization of treatment strategies in cases of secondary trigeminal neuralgias because of tumor compression. Research managed to improve treatment guidelines and protocols for management of patients with hyperactive dysfunctions of cranial nerves, such as trigeminal, intermediate and glossopharyngeal neuralgias, hemifacial spasm, vestibular paroxysmia and pulsatile tinnitus, severe paroxysmal arterial hypertension and other conditions caused by neurovascular conflict (GRADE criteria were used).Long-term outcome follow-up multifactorial analysis of trigeminal neuralgias treated by microvascular decompression (161 cases with median observation time of 70 month (5,8 years)) showed that according to our data the sole prognostic factor for failed treatment (pain recurrence or worsening of achieved effect) was symptoms duration from onset to surgery time.
