Object of study - patients with hereditary microspherocytic anemia complicated by gallbladder lithiasis, who underwent splenectomy simultaneously with cholecystolithotomy The aim of the study is to determine the effectiveness of organ-sparing cholecystolithotomy on the gallbladder in patients with hereditary spherocytosis (SSC) complicated by gallbladder calculi along with splenectomy and long-term results to develop a modern concept of surgical treatment of this category of patients. Research methods - clinical, cytological, histological, radiological, sonographic, immunophenotypic studies, computed tomography. In 4 patients with SSC complicated by gallbladder calculi, organ-sparing cholecystolithotomy was performed, which included cholecystotomy and lithoextraction along with splenectomy. A retrospective analysis of the long-term results of splenectomy and cholecystolithotomy in 18 patients with SSC was performed. Our experience indicates the need for meticulous selection of patients for this operation. Indications for organ-sparing surgery on the gallbladder are the complete absence of inflammatory (acute) changes in the gallbladder wall, patency of the gallbladder duct and the presence of exclusively pigmented stones. A fundamentally new approach to the treatment of uncomplicated gallbladder calculi with SSC has been developed - the method of cholecystotomy with lithoextraction, the advantage of which is the preservation of physiological function of the bile ducts. Based on the results of the research, an information letter "Treatment tactics for hereditary spherocytosis complicated by gallbladder calculi" was prepared.
