A protocol for examination of patients with rheocclusions after open and endovascular revascularizations has been developed. Based on morphological studies, the main factors of arterial reocclusion were determined - progression of atherosclerotic lesions distal or proximal to the reconstructed segment (38 (40.9%)), false anastomotic aneurysms (9 (9.7%)), thrombosis (17 (18.3%)), hyperplasia (neoplasms). shunt (29 (31.2%) patients). Gene expression analysis was performed in samples from patients with a confirmed diagnosis of atherosclerosis with reocclusion after revascularization. The set of genes taken for analysis was selected according to the available literature on the basis of their possible association with atherosclerosis and consisted of the following genes: G6PG, VEGF, TGF-beta, APOE, TNF-beta. The obtained results showed the presence of differential expression of some genes (VEGF, TGF-beta) from the set in clinical samples, in almost all samples the presence of APOE gene mRNA was not identified. The frequency of neointimal hyperplasia in different types of shunts has been studied. A differentiated approach to the choice of revascularization method depending on the cause of reocclusion and localization of the lesion has been developed. A patent was obtained for a method of treating reocclusion of the femoral-popliteal segment. Developed and after conducting research in animal experiments (rabbits) introduced into clinical practice methods for the prevention of neointimal hyperplasia in autovenous shunts in patients during primary revascularization, including one with treatment of the inner surface of the autovenous shunt with a solution of cytostatics, which has no analogues.
