The purpose of the study: to increase the effectiveness of treatment of patients with locally advanced colorectal cancer with multivisceral surgery intervention through early correction of peri- and postoperative complications with taking into account factors of inflammation and aggressiveness of the tumor process. Research methods and equipment: clinical-instrumental, immunoenzymatic, electron microscopic, statistical; immunoenzyme analyzer Immunochem-2100, sled microtome MC-2. Theoretical and practical results: new data were obtained regarding the complex of prognostic factors of the course of the peri- and postoperative period in patients with LACRC. Supplemented scientific data on the treatment of patients with LACRC after multivisceral resections in the peri- and postoperative period, and also determined the personalized volume of surgical treatment with in these limits. The morphological features of the wound process after multiorgan combined interventions for primary locally advanced colorectal tumors and their recurrences have been established. Novelty: for the first time, the prognostic significance of the markers VEGF, COX-2, PGE, CRP, PCT in blood serum for the formation of a risk group for the occurrence of postoperative complications in patients with locally advanced colorectal cancer has been established. Effectiveness of implementation: increasing the effectiveness of therapy for patients with locally disseminated tumors of the pelvic organs.
