The purpose of the work: increase effectiveness of treatment of insulin-resistant patients with papillary thyroid cancer by the means of development of new personalized technologies for complex antitumor treatment using metformin. Methods of research and equipment: clinical instrumental, immunoferment, radiometric, dosimetric, statistical; immunoassay analyzer Immunochem-2100, dosimeter-radiometer MKS-05 «Terra», spectral chemiluminescent immunoanalyzer-photometer«COBAS e411»; «COBAS 311», immunofluorescent analyzer-photometer «АІА-2000»; immunoferment microplate analyzer «SUNRISE TECAN». Theoretical and practical results: the effectiveness of complex treatment using metformin in insulin-resistant patients with papillary thyroid cancer was assessed (specificity – 75 %, sensitivity – 80 %). Threshold value of insulin-like growth factor-2 (IGF-2) was established – 707 pg/ml which is significant predictive for the development risk of metastatic lesions of papillary thyroid cancer. Novelty: the use of metformin in the complex treatment of insulin-resistant patients with papillary thyroid cancer was substantiated, as well as performance of suppressive hormone therapy considering the levels of thyreotrophin and reverse T3 at the stage of posttreatment observation. The program of personalized complex treatment of insulin-resistant patients with papillary thyroid cancer using metformin was developed. Recommendations on treatment and observation of insulin-resistant patients with papillary thyroid cancer were suggested, as well as substantiation of a complex of diagnostical, treatment and prognostic measures for individualization of therapy for insulin-resistant patients with papillary thyroid cancer taking into account tumor aggression and metabolic disorders. Effectiveness of implementation: decrease in expenditures on treating recurrences.
