The purpose of the work is to determine the features of the regulation of anti-inflammatory and pro-inflammatory cytokines taking into account the course of plasma cell myeloma and to investigate the distribution of polymorphic variants of cytokine genes in patients who suffered after the result of the Chornobyl nuclear power plant accident. Research methods are clinical, molecular genetic, immunogenetic, statistical. The nature of cytokine regulation in PCM patients was evaluated depending on the stage and course of the disease, taking into account contact with ionizing radiation (IR) in the anamnesis. There were examined 74 patients, including 35 irradiated patients. A probable increase in the frequency of the TGF-β1 codon 10 T/T genotype (0.591, p < 0.05) in PCM patients (irradiated, non-irradiated individuals) relative to population controls indicates a unidirectional association with the risk of the disease and provides a reason to consider this single-nucleotide polymorphism as an independent immunogenetic factor of predisposition to PCM development. The relationship between the changes in the production of the studied cytokines and the course of PCM was revealed (normalization of general clinical and biochemical indicators after 3 and 6 months of treatment): in patients who achieved remission, significantly reduced concentrations of IL-6, TNF-α and IL-10 were registered compared to the indicators patients with disease progression, which gives reason to attribute these factors to prognostic criteria for the course of PCM. At the same time, in the irradiatedof PCM patients takes placea probable difference in the degree of cytokine imbalance, which confirms the certain importance of IR in the pathogenesis of the disease as a possible predictor of the formation and aggravated course of the disease.
