Yanovsky National academy of medical sciences of Ukraine" popup.description2  Study object – 250 patients with newly diagnosed pulmonary sarcoidosis after monotherapy and combined treatment with methylprednisolone (MP) and methotrexate (MTX). Aim – to study the effectiveness and tolerability of treatment of MP in combination with MTX in patients with pulmonary sarcoidosis refractory to monotherapy of MP and to evaluate long-term outcomes for at least 6 months after achieving clinical remission. Methods – clinical, computed tomography of chest, pulmonary ventilation and diffusion diagnostic tests. It was established that resistance to GCS therapy was noted in 55 (28,9 %) patients, cases of unsuccessful MTX therapy were observed less frequenty (7 patients – 11,7 %), mainly in patients with macronodular lesions of the parenchyma. The combined therapy of MP (12 mg/day) and MTX (10 mg/week) in 55 patients with insufficient effectiveness of GCS allowed to achieve clinical remission with normalization of CT data in 35 patients, refractory sarcoidosis was confirmed in 20 cases - slight clinical improvement with incomplete CT regression – a change in the lungs. It was determined, that out of 35 patients with remission of sarcoidosis during 12 months of observation, 19 had relapses of the disease, which is 54,3 %. It was established, that in Ukraine the refractory pulmonary sarcoidosis is observed in 10,8 % of patients, which is consistent with the data of foreign literature. It was determined that in patients with refractory pulmonary sarcoidosis, the III stage of the disease was significantly more often observed in 14,8% of people, while in patients with a successful treatment of the disease, its frequency was 4 %, extrapulmonary lesions also occurred more often in 25,9 %, compared with patients with a successful treatment of the disease – 11,2%.
