Kolomiychenko Institute of Otolaryngology of National Academy of Medical Science of Ukraine" popup.description2  A comprehensive clinical and imaging examination of patients with neoplasms of the oropharynx and parapharyngeal space was conducted during 2020-2022. 98 patients were examined, from which 44 patients were selected, whose data were collected in the dynamics of observation. All patients received a complex of imaging methods of examination (spiral CT with contrast and MRI-perfusionography), thanks to which the location of the neoplasm, its relationship to the vascular and nerve bundle of the neck and surrounding structures, and the probable nature of the neoplasm were determined. Owing to the obtained results of the examination (poststyloid location, suspicion of malignant process, intimate location with ICA), were decided to use external access in 8 patients. We studied the localisation of the neoplasm in relation to the muscles of the pharynx. It was noticed that m. Palatoglossus in all cases (100%) was located in front of the neoplasm, and in 42.9% of cases it was located at a distance from the neoplasm (3-13 mm). m. Palatopharyngeus was also located in front of the neoplasm in 35.7% of cases, in another 37.5% of cases it was located medially. Regarding this, a modified transoral method of access to neoplasms of the parapharyngeal space was developed, where the dissection is carried out along the posterior palatine bracket, thus leaving the muscles of the palate intact. The assessment of quality of life in patients after using a modified transoral method of approach showed a statistically significantly lower number of points (p<0.05). It was observed in the postoperative period, compared to the group of patients with a classic transoral approach. Equalization of the difference in the quality of life in both groups of patients was observed 6 months after surgery (p>0.05).
