Yanovsky National academy of medical sciences of Ukraine" popup.description2 The object of the study is 426 patients with resistant forms of tuberculosis of the lungs and pleura (ML-TB and SHLS-TB) who were operated on using resection, video-assisted or collapsosurgical techniques. The aim of the work is to increase the effectiveness of surgical treatment of patients with multidrug-resistant pulmonary tuberculosis and pulmonary tuberculosis with widespread drug resistance. Research methods are clinical-instrumental, laboratory, cytological, microbiological, pathomorphological, statistical. It was determined that the optimal term of operative treatment of patients with MDL-TB and SLS-TB of the lungs is 4–6 months. from the beginning of PTT according to the 4th category. The use of collapsotherapeutic methods improves the dynamics of clinical indicators by 19.4 %, radiological indicators by 12.1 %, normalizes body temperature by 6.1%, reduces the number of postoperative complications by 6.4%, and lowers the mortality rate by 2.2 %; increase the overall effectiveness of treatment by 8.6 %. It was established that performing lung resections with corrective TP makes it possible to achieve a long-term, stable reduction in hemithorax volume, increase the efficiency of the operation by 16.1 %, and reduce postoperative mortality by 10.1 %. It was determined that among resection interventions the most effective are lobectomy and segmental resection, the use of which allows to increase the effectiveness of treatment on 7.3 %, 1.9 times lower the mortality rate, 8.0 % lower the level of postoperative complications, 1.9 times lower the frequency of bronchial fistulas. The technology of surgical treatment of patients with resistant pulmonary tuberculosis was developed made it possible to achieve the efficiency of primary extrapleural thoracoplasty by 82.1 %, increase the efficiency of surgical treatment of patients by 12.9 %, and reduce the level of postoperative mortality by 11.8 %.
