Yanovsky National academy of medical sciences of Ukraine" popup.description2  Object of the study – 627 children with newly diagnosed tuberculosis (TB) from established and unknown contacts, statistical indices of the Ministry of Health of Ukraine regarding TB in children. The aim of the study is to improve the detection of tuberculosis infection and dispensary surveillance of children who have been in contact with patients with tuberculosis. Research methods: clinical-radiological, laboratory, statistical. It was established that the incidence of TB in children in 2021 was 7.2 per 100 thousand. Over the past 16 years, the percentage of bacterial shedders among sick children under 14 has increased by 2.7 times, among adolescents by 2.4 times. The structure of clinical forms of TB has deteriorated due to the growth of common forms with MDR+ and destruction, drug-resistant TB. The incidence of contact children exceeds the incidence in the population by almost 250 times. It was found that 61.7% of the examined children are from unknown contacts, and 81.3% are in contact with a patient with MDR+. Contact with the mother (27.1%) and father (25.0%) occurs most often. It has been determined that 39.2% of patients from TB foci are detected after seeking medical help, and 46.2% of children from contacts have contact with patients with XDR-TB. It was found that children from contacts are more likely to infiltrative pulmonary TB (24.1%) and primary tuberculosis complex (20.7%), from unidentified contacts – bronchial TB (36.7%) and infiltrative pulmonary TB (27.8%). Most of the examined children have no bacterial excretion (60.6%), no destruction (73.7%), and 68.7% have limited processes. The technology of managing children in contact with a TB patient regardless of bacterial excretion by timely and thorough examination, termination of contact and prescription of preventive treatment is proposed, which will contribute to the early detection of TB infection and reduce the risk of local TB in children.
