The object of the study is patients with hereditary microspherocytic anemia complicated by gallstone lithiasis, who underwent splenectomy simultaneously with cholecystolithotomy The purpose of the work is to find out the effectiveness of the organ-preserving operation of cholecystolithotomy on the gallbladder in patients with hereditary spherocytosis (HCC) complicated by gall bladder calculus at the same time as splenectomy and, based on the long-term results, to develop a modern concept for surgical treatment of this category of patients. Research methods - clinical, cytological, histological, X-ray, sonographic, immunophenotypic studies, computer tomography. In 4 patients with CC complicated by gall bladder calculus, an organ-preserving operation of cholecystolithotomy was performed, which included, along with splenectomy, cholecystotomy and lithoextraction. A retrospective analysis of long-term results of splenectomy and cholecystolithotomy in 18 patients with SCC was conducted. Our experience indicates the need for careful selection of patients for this operation. Indications for organ-sparing surgery on the gallbladder are the complete absence of inflammatory (acute) changes in the gallbladder wall, the patency of the cystic duct, and the presence of exclusively pigment stones. A fundamentally new approach to the treatment of uncomplicated calculus of the gall bladder in SCC was developed - a method of cholecystotomy with lithoextraction, the advantage of which is the preservation of the physiological function of the biliary tract.
