Peripheral blood cells and bone marrow cells of 38 polycythemia vera patients, 30 essential thrombocythemia and 62 myelofibrosis have been investigated. The aim of this study was to establish the role of immunomodulating cytokines and growth factors in the course of certain myeloproliferative neoplasms, particularly at disease progression and development of hemostasis disorders. Another purpose of the study was to create pathogenetically justified treatment approaches. Methods: general clinical, histological, molecular-genetic, coagulation investigations, statistical analysis. Comparison of the known prognostic factors used for risk profiling of patients with MF, PV and ET was performed within the third stage of this study. The pattern of their changes was found to be similar in PV, ET and MF whereas certain differences were caused by molecular-genetic alterations of the substrate cells. In majority of the PV patients changes in TNF, IL-6, IL-8 and VEGF were detected as compared to the control group. The direct correlation was confirmed between platelet levels and VEGF and TNF concentrations as well as between platelet and white blood cell levels and IL-6, IL-8 concentrations. In patients with ET the highest level of VEGF was detected. On the other hand no difference in concentrations of this cytokine was found between JAK2-positive and JAK2-negative patients. Platelet levels in this group of patients showed correlation with concentrations of TNF and IL-8. Significantly increased levels of proangiogenic and proinflammatory cytokines were revealed in MF. Connection between VEGF levels and risk degree according to the DIPSS has been detected: patients with the low levels of this cytokine belonged to the higher risk group which is an indirect evidence of increasing bone marrow fibrosis.
