Since radiotherapy remains an essential part of the multimodal treatment of patients with cancer, the necessity to increase the effectiveness of radiotherapy for GBM patients is clearly evident. Our study has shown the feasibility of hypofractionated radiotherapy as an alternative to standard adjuvant radiotherapy for patients with primary GBM and stereotactic radiosurgery for recurrent GBM. The search for new solutions to one of the main tasks of radiotherapy, as is the extension of the therapeutic interval of radiosensitivity of tumor tissues, is currently based on the investigation of their individual radiosensitivity. Molecular genetic examinations were performed for the individual radiosensitivity assessment using the classical gel electrophoresis-based method (Comet assay) that can be used to measure DNA damage. Advanced knowledge of the immune response mechanisms has contributed to improving of immunotherapeutic approaches in neuro-oncology aimed at inducing the elimination of residual tumor cells by the patient's immune system after surgical treatment. Among these approaches are therapeutic vaccines, which are developed to stimulate the immune system against tumor-specific antigens mainly through the induction of cytotoxic T-lymphocytes. Our study has demonstrated the feasibility of using a vaccine based on autologous dendritic cells (DC) in the adjuvant treatment of GBM patients. The most informative immunological criteria for the effectiveness of DC-based immunotherapy for GBM patients were determined. The DC-based immunotherapy does not significantly change the quantitative parameters of the main peripheral blood populations (T-, B- and NK-lymphocytes).
